You might want to check the idler pulley to make sure it isn't pushing the belt off. The idler bracket can be adjusted so the pulley is pushing straight down on the pulley. Another thing to look for is the wear on the idler pulley, if there is a small grove worn on the pulley that can push the belt off. This is presuming the pulleys are in good alignment. you will not like this answer to your problem if the idler does not cure. take the pivot shaft completly apart and put a square from frame to the pivot shaft. or even take tire off and see if pivot shaft is bent. good luck with the idler.

Check to make sure the cylinder pulley is not on wrong side out.If I remember it should be dished in.
